A: 2: You had minimum high-card values for your first response, and your partner's second bid hasn't improved your hand. Bid two spades, inviting game. In the method many pairs employ, a two-over-one response is forcing to game. In that style, a response of two clubs would be aggressive but not unreasonable.

A: Pass: This is an awkward problem in "Standard" methods. Your partner's minimum change of suit might conceal a hand worth almost a game-force, and you might miss a game if you don't act again. Nevertheless, partner may have minimum values. Pass. Your hand isn't strong enough to bid either 2NT or three hearts.

A: 2: Your hand is worth inviting game but not forcing to game. Bid two diamonds, planning to raise a rebid of two hearts to three. A response of one spade might locate a 4-4 fit, but a 5-3 heart fit may play as well or better. Partner will be able to judge your prospects more accurately if you bid the side suit where your values lie.
